Abstract:

Background: Male reproductive health is established during fetal life and continues to evolve throughout childhood and adolescence. Increasing evidence demonstrates that many cases of adult male infertility originate from pediatric conditions that are potentially preventable or treatable if recognized early. Despite advances in pediatric healthcare, opportunities to preserve future fertility are frequently overlooked due to delayed diagnosis, inadequate follow-up, and limited awareness regarding the long-term reproductive consequences of childhood diseases.

Methods: This invited educational lecture presents a comprehensive narrative review of current literature and international guideline recommendations from major professional organizations, including the European Association of Urology, American Urological Association, American Society for Reproductive Medicine, and the European Society for Paediatric Urology. Evidence regarding the diagnosis, management, fertility preservation, and long-term reproductive outcomes of common pediatric urological and endocrine disorders will be synthesized alongside illustrative clinical cases.

Results: The presentation will highlight the impact of cryptorchidism, hypospadias, adolescent varicocele, testicular torsion, childhood genitourinary infections, endocrine disorders, and pediatric cancer therapies on adult fertility potential. Emerging concepts including developmental origins of reproductive health, environmental endocrine disruptors, fertility preservation in prepubertal boys, and the transition from pediatric to adult reproductive care will also be discussed. Current evidence supports early intervention, individualized surveillance, and multidisciplinary management as key determinants of improved reproductive outcomes.

Conclusion: Protecting future fertility begins during fetal development and childhood rather than in adulthood. Early recognition of pediatric conditions affecting the testes and reproductive tract, adherence to evidence-based management, and coordinated lifelong follow-up can substantially improve future fertility, endocrine health, and quality of life. Integrating pediatric, surgical, endocrine, and reproductive expertise provides an opportunity to transition from treating infertility to preventing it, ultimately advancing lifelong male reproductive health.

Dr. Anil Kumar Sah is a Consultant Urologist at Nepal Mediciti Hospital, Nepal, and a Fellow of the European Board of Urology (FEBU). He is currently pursuing advanced fellowship training in Andrology, Sexual Medicine, and Male Infertility at VNA Hospital, New Delhi. A Gold Medalist in both his MBBS and Health Assistant training, Dr. Sah has authored over 20 peer-reviewed publications and several medical textbooks. He serves as a peer reviewer for leading international journals, including Springer Nature and BMC Urology, and is actively involved in academic teaching, research, and international urological education across South Asia
